5-step action plan aims to bridge psoriasis, metabolic disease gap

Posted on August 6, 2026

The International Psoriasis Council has released a list of five recommendations for prioritized action items that will help dermatologists incorporate metabolic disease considerations into psoriasis care.
Researchers attending the 2025 International Psoriasis Council Think Tank symposium identified metabolic disease management as the primary unmet need and future priority of psoriasis treatment, according to a summary paper published in the Journal of the American Academy of Dermatology. Based on feedback from more than 150 survey responses from dermatologists, the five finalized action items

The Psoriasis-Metabolic Disease Nexus: A Critical Call to Action for Dermatologists

As medical professionals, we are continually refining our understanding of complex diseases, moving beyond organ-specific silos towards a more holistic, patient-centered approach. The recent initiative by the International Psoriasis Council (IPC) to release a five-step action plan for integrating metabolic disease considerations into psoriasis care represents a pivotal moment in this evolution. It underscores a fundamental shift in how we view and manage psoriasis – not merely as a skin condition, but as a systemic inflammatory disease with far-reaching implications for overall health.

For too long, the visible manifestations of psoriasis have often overshadowed its less apparent, yet equally dangerous, internal impact. The IPC’s identification of metabolic disease management as a primary unmet need highlights a critical gap in current clinical practice. This isn’t just about adding more tasks to a dermatologist’s already busy schedule; it’s about fundamentally reshaping our approach to optimize patient outcomes, reduce long-term morbidity, and enhance the quality of life for individuals living with this chronic condition.

While the specific five recommendations are not detailed in the summary, their intent is clear: to equip dermatologists with the tools and framework necessary to proactively address the heightened risk of metabolic comorbidities in their psoriasis patients. This will likely involve strategies for systematic screening, appropriate referral pathways, enhanced patient education, and fostering interdisciplinary collaboration. By proactively integrating metabolic health into psoriasis management, we move closer to providing truly comprehensive care that addresses the whole patient, not just their skin.

Background: Unpacking the Psoriasis-Metabolic Link

Psoriasis is a chronic, immune-mediated inflammatory disease predominantly affecting the skin, characterized by well-demarcated erythematous plaques covered with silvery scales. It can also involve the joints, leading to psoriatic arthritis, and nails. Beyond these visible manifestations, compelling evidence has emerged over the past two decades demonstrating that psoriasis is a systemic inflammatory disorder associated with a significantly increased risk of various comorbidities, particularly those grouped under the umbrella of metabolic syndrome.

Metabolic syndrome is a cluster of conditions that occur together, increasing an individual’s risk of heart disease, stroke, and type 2 diabetes. These conditions include:

  • Increased blood pressure (hypertension)
  • High blood sugar (insulin resistance, impaired glucose tolerance, or diabetes)
  • Excess body fat around the waist (abdominal obesity)
  • Abnormal cholesterol or triglyceride levels (dyslipidemia)

The link between psoriasis and metabolic syndrome is not coincidental; it is driven by shared inflammatory pathways. Chronic systemic inflammation, a hallmark of moderate-to-severe psoriasis, contributes to insulin resistance, endothelial dysfunction, and accelerated atherosclerosis. Pro-inflammatory cytokines like TNF-alpha, IL-17, and IL-23, which are central to psoriasis pathogenesis, also play roles in the development and progression of metabolic dysfunction. This creates a “psoriatic march,” where the persistent inflammatory state predisposes individuals to a cascade of cardiovascular and metabolic complications, including myocardial infarction, stroke, and fatty liver disease.

Historically, dermatologists have focused primarily on managing cutaneous symptoms and psoriatic arthritis. However, the recognition of psoriasis as a multisystem disease necessitates an expansion of this traditional scope. Patients with moderate-to-severe psoriasis have a higher prevalence and earlier onset of metabolic syndrome, type 2 diabetes, obesity, dyslipidemia, and hypertension compared to the general population. This elevated risk translates into higher rates of cardiovascular events and increased mortality, underscoring the urgency of integrating metabolic health surveillance into routine dermatological practice.

Why This Initiative Matters: A Doctor’s Perspective

From a medical doctor’s standpoint, the IPC’s action plan is not just timely but essential for several critical reasons:

1. Improved Patient Outcomes and Reduced Morbidity

This is, without a doubt, the paramount reason. Proactive screening and early detection of metabolic comorbidities can significantly alter a patient’s long-term health trajectory. Identifying conditions like hypertension, dyslipidemia, or pre-diabetes at an early stage allows for timely interventions, including lifestyle modifications and pharmacological treatments. This can prevent or delay the onset of severe cardiovascular events, reduce the risk of type 2 diabetes progression, and ultimately extend lives. For a dermatologist, expanding our focus means offering truly comprehensive care that protects patients from risks beyond their skin.

2. Holistic Patient Care and Quality of Life

Addressing the whole patient, rather than just their visible symptoms, leads to a more robust and effective treatment strategy. Patients with psoriasis often face significant psychological burden and reduced quality of life. The knowledge that their dermatologist is also attending to their overall health, including their heart and metabolic well-being, fosters trust and reinforces the idea that their condition is being managed comprehensively. It also streamlines care, potentially reducing the need for multiple specialist visits for initial screening, thereby easing the patient’s healthcare journey.

3. Elevating the Role of the Dermatologist

This initiative solidifies the dermatologist’s position as a key player in chronic disease management. While primary care physicians are crucial, dermatologists often have consistent, long-term contact with psoriasis patients. This provides a unique opportunity for ongoing surveillance and intervention. Embracing this broader role requires dermatologists to enhance their knowledge in metabolic health, comfortable discussing lifestyle interventions, ordering relevant labs (e.g., lipid panels, glucose, HbA1c), and making appropriate referrals to primary care, cardiology, or endocrinology specialists. It empowers us to be more than just “skin doctors,” but rather guardians of our patients’ overall systemic health.

4. Fostering Interdisciplinary Collaboration

Effective management of psoriasis with metabolic comorbidities demands seamless communication and collaboration among different medical specialties. The action plan will likely emphasize the creation of clear referral pathways and communication protocols between dermatologists, primary care physicians, cardiologists, endocrinologists, dietitians, and other allied health professionals. This ensures a coordinated approach to patient care, leveraging the expertise of each specialist to achieve the best possible outcomes.

5. Significant Public Health Impact and Economic Benefits

Psoriasis affects millions worldwide. By systematically integrating metabolic screening and management into psoriasis care, we can significantly reduce the overall burden of cardiovascular disease and diabetes in this high-risk population. Proactive management and prevention of complications can lead to substantial long-term healthcare cost savings by averting expensive hospitalizations, procedures, and treatments associated with advanced cardiovascular disease and diabetes-related complications.

In conclusion, the IPC’s five-step action plan is a crucial step forward in modern dermatology. It reflects a maturing understanding of psoriasis as a systemic disease and a commitment to providing truly holistic, patient-centered care. As medical professionals, embracing these recommendations will not only enhance our clinical practice but, more importantly, will profoundly improve the health and well-being of our psoriasis patients, offering them a future where their skin condition does not dictate their cardiovascular destiny.
